Have your say on heart failure services

Complete a short survey

NHS Sussex are currently running a survey for adults living with heart failure and those who care for them.

They want to improve their heart failure services so that people can access high-quality care across the whole of Sussex. To help them make the right changes, they are keen to understand your experiences and hear what you think a good service would look like.

Sussex Health and Care brings together hospital and community NHS Trusts, general practices, social care, and others to plan and deliver services that meet the needs of the people of Sussex.

 

If you live with heart failure or are a carer or family member of someone who does, please complete the following survey by 8th September 2023 and let NHS Sussex know your thoughts.

Join an online focus group

NHS Sussex are currently running a series of online focus groups for service users to enable us to talk directly to local people about what matters to them and discuss ideas for improvement in more detail.

Have a telephone interview

If you are unable to attend one of the online focus groups, NHS Sussex are running telephone interviews.

Telephone interviews are available on 5th October 2023 and 10th October 2023.
