Join in the Connecting 4 You ‘Big Health and Care Conversation’.
Connecting 4 You (C4Y) is a partnership of health and social care organisations working together to help improve local health and social care for the population of High Weald Lewes Havens. It is led by High Weald Lewes Havens Clinical Commissioning Group (CCG) and East Sussex County Council, alongside Community and Mental Health Trusts, working in partnership with Healthwatch, patients, carers and the local community and voluntary sector.
We began the Big Health and Care Conversation’ with an event in Newhaven in April this year, and since then have been having smaller conversations with local people about health and care.
They are holding another Big Conversation event on 31st October 2018 at the Town Hall in Lewes, from 10am to 1pm.

The C4Y Partnership is hosting the ‘Big Health and Care Conversation’ to give you the chance to meet our commissioners and clinical leaders to discuss your priorities for health and care in the area, including mental health, primary care, frailty and urgent care. In addition, there will be an opportunity to hear about and discuss the regional Sustainability and Transformation Partnership (STP) and the unprecedented financial challenges facing the health and social care system.
We want to hear from you about your views and experiences and your suggestions for improvement of local health and care services in Lewes, High Weald and surrounding areas, and we also want to talk to you about how local communities can support the health and wellbeing of local people.
