New community diabetes service to launch

June 21, 2016

Sussex Community NHS Foundation Trust are pleased to announce that from 11th July they will be providing a new consultant-led community diabetes service in Brighton & Hove and High Weald Lewes Havens for people aged 18+ living with Type 1 and Type 2 diabetes.

The team will provide expert specialist diabetes care and support to help people manage their diabetes more effectively closer to home. This will mean fewer people will need to attend an acute hospital for specialist outpatient diabetes services.

Richard Quirk, Medical Director, said “Our service, called Diabetes Care for You, will help people to live well with diabetes by offering advice, support and diabetes management in a range of new locations. Our teams are working hard to ensure the safe transfer for patients and for staff who are joining our SCFT family. We will continue to work together with both CCGs, GPs, our partners and other local providers to improve health and care outcomes for people living with diabetes in Brighton & Hove and High Weald Lewes Havens.”

More information about the service is available from our website: www.sussexcommunity.nhs.uk/diabetesc4u.
